UPDATE: Format for Refund Application provided below the table
The Indian Railways has changed rules for refund of tickets to help passengers claim refunds easily. These rules are applicable for trains that were or will be cancelled between 21st March till further orders, or resumption of normal train services, as per a circular issued by the Railway Board.
Refund for physical / PRS counter tickets can now be obtained for a longer window from the date of journey. If passengers have already cancelled their tickets, and cancellation fees were charged, they can apply for a refund of such fees also. Scroll down for more details.
New Refund Rules for Lockdown Cancellation of Indian Railways Services

I am from Pune. I have cancelled the paper tickets for the journey on 25 March at the counter. The PNR number is 4512769521. I got the amount deducting the cancellation charges. Now it is not clear how to file the TDR as the tickets are not booked on IRCTC site.
Further to whom I have to approach for refund either at Pune or mumbay chief commercial officer. This is general problem.

I am really sorry to inform that I have visited the CCM 2 times and simply they have accepted
My application. Three months over . In the meantime I referred the case to next higher authority and even sent a reminder. No action so far. On my query at CCM office they told that lot of application are pending and will take long time. Hope the best.
My ticket date is 23 04 2020 in lockdown period I went to railway station a officer Said to me when Will the railway counter open u come to cancel the ticket then I go to the station Oct 25 ticket issuing officer Said to me the ticket is not cancelled it was lapsed total amount of the ticket is 4000 rupees.please give me a answer what shall I do?

WHEN I APPROACHED COUNTER FOR TICKET CANCELLATION, THEY SAID IT CAN NOT BE CANCEL AS SIX MONTHS IS PASSED
WHEN YOUR OWN COUNTER WAS CLOSED FOR MOTE THAN SIX MONTHS, HOW CAN ABOVE RULE APPLICABLE TO US?
